Job Summary
Plan, coordinate, and manage care for complex patients and their families throughout the continuum of care. Execute effective case management functions by procuring and negotiating appropriate health care services. Assist patients in utilizing care resources while performing utilization management and discharge planning duties. Manage chronic disease case management based on inpatient settings. Requires a California Registered Nurse License, recent post-graduate case management training, and two years of acute care or one year of case management experience. Work 8-hour day shifts on a per-diem schedule at Providence Queen of the Valley Medical Center in Napa, CA.
Required Qualifications
- Recent completion of a post-graduate level curriculum focusing on case management
- Current clinical experience with 2 years clinical experience in an acute care hospital setting
- 1 year current experience in an inpatient or outpatient case management setting
- California Registered Nurse License
- 2 years Case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment
