Job Summary
Care Manager RN per diem at Providence St. Joseph Medical Center in Burbank, CA. The role supports case management through collaboration with patients, caregivers, nurses, social workers, physicians, payers, and the community to facilitate communication and care coordination along a continuum and transitional care management, with emphasis on the patient’s right to self-determination, social determinants of health, and comprehensive care. Requires RN license in California, a BSN preferred (or equivalent education/experience with a plan to obtain the degree within 3 years), and at least 2 years of healthcare experience and 2 years in case management (or TIP program). Preferred certifications include ACM or CCM.
Required Qualifications
- Bachelor's Degree in Nursing preferred or equivalent educ/experience
- California Registered Nurse License upon hire
- 2 years Experience in Healthcare related field (Acute, Ambulatory, Post-Acute, etc.)
- 2 years Experience in Case Management (Care Coordination or Utilization Management) or TIP program experience
- Certification in Case Management (ACM or CCM) preferred
