Care Manager RN Outpatient

Job Summary
Manage a panel of high-risk, high-cost patients with chronic conditions by assessing needs, developing individualized care plans, and collaborating with physicians to coordinate transitions across acute, rehab, skilled nursing, and community settings. Coach patients on disease management and self-care strategies while connecting them with community resources to overcome barriers to care and improve health outcomes. Monitor progress and adjust care plans using evidence-based practices to ensure optimal results. Requires active Maryland or NLC RN license, BLS/CPR, and Care/Case Management certification; BSN with 5 years acute care experience or ASN with 10 years.
Required Qualifications
- Associate Degree in Nursing (ASN)
- Bachelor of Science in Nursing (BSN)
- Active Registered Nurse license in Maryland or qualifying Nurse Licensure Compact (NLC) multi-state license
- Current BLS/CPR certification
- Ability to obtain active Pennsylvania and West Virginia RN licenses within 6 months of employment
- Care/Case Management Certification required within 5 years of employment
- 10 years of acute care nursing experience
- 5 years of acute care nursing experience
Desired Qualifications
- Bachelor of Science in Nursing (BSN)
- Care management, case management, managed care, patient-centered medical home, or integrated care management experience
- Experience working with chronic and complex patient populations
