Care Manager (RN) - Oncology

Job Summary
The PCRM (Care Manager RN) coordinates patient care from pre-admission through post-discharge, using the nursing process to manage the plan of care, identify and remove barriers, and ensure efficient resource utilization. Collaborates with physicians, nurses, and other health care workers to manage transitions to the next level of care, and serves as the central information source for third-party payers to communicate the plan of care and support approvals for post-discharge care. Requires RN licensure with a Bachelor’s degree (Masters preferred) and relevant clinical experience in oncology care; engages with evidence-based criteria to ensure appropriate levels of care and timely patient decisions.

- Bachelor’s degree (RN licensure required)
- RN licensure required
- Masters preferred
- five years recent clinical experience applicable to the population preferred
- knowledge of clinical practice guidelines
- experience with computer databases
- knowledge of payer protocols
