Care Manager RN - MSO CM Heritage Per Diem

Care Manager RN provides patient advocacy and coordinates care across the continuum to help patients achieve optimal health, access to care, and appropriate utilization of resources. The role utilizes assessment, planning, implementation, coordination, monitoring, and evaluation of care plans, collaborating with nurses, social workers, physicians, and other practitioners to ensure effective resource coordination and seamless care delivery. The position aligns with Providence’s mission of patient-focused, whole-person care and supports a multidisciplinary approach within Providence Medical Foundation across multiple care networks.
