Job Summary
Care Manager RN delivers in-home CKD/ESRD/dialysis care, builds trusting patient relationships, and collaborates with a care team to develop and adapt individualized care plans. Responsibilities include in-home care visits, monitoring biometric data, medication reconciliation, patient health assessments, education on CKD/ESRD and associated comorbidities, promoting adherence to treatments, engaging family and social supports, serving as the primary point of contact, facilitating proactive care discussions (including Advance Care Plans), obtaining vitals during visits, onboarding new patients, and supporting care transitions with dialysis providers. Requires frequent local travel; hybrid in-home/office schedule; BLS certification; RN license in the state of the posted location; 2+ years in care management with CKD/ESRD; ability to take some call remotely; strong communication and proficiency with Microsoft Office and mobile/web apps.
Required Qualifications
- Graduate of an accredited School of Nursing
- Currently licensed as a Registered Nurse in the State of the posted location
- 2+ years experience in care management and/or with CKD/ESRD patients
- Basic Life Support (BLS) certification
- Ability to travel locally for in-home visits
