Care Manager RN - Managed Care Support

Manage patient populations with chronic illness and co-morbidities by coordinating with multiple primary care physicians and serving as the central link to identified patient groups. Conduct skilled assessments, provide education, and collaborate to coordinate healthcare and community resources, enabling patients to develop self-management skills and achieve optimal functional health status. Assist patients, families, and systems in attaining high-quality, evidence-based, cost-effective, and patient-focused outcomes while working under minimal supervision within established policies and the nursing code of ethics.
