Care Manager RN - Managed Care Support

Manage patient populations with chronic illness and multiple co-morbidities by coordinating with multiple PCPs and serving as the central link to identified patient populations. Conduct skilled assessments, education, and collaboration to coordinate healthcare and community resources, assisting patients in gaining self-efficacy and achieving optimum functional health status. Achieve high quality, evidence-based, cost-effective, and patient-focused outcomes while working under minimal supervision within general policies and the nursing code of ethics. May perform other ambulatory care RN duties as required. This role supports the Medical Home Model within Providence Medical Group Northwest Washington.
