Care Manager (RN) - Inpatient

Clinical Case Manager (RN) in inpatient setting coordinates patient care within a multidisciplinary team to ensure efficient, effective, and safe transitions through the health care system. Responsibilities include monitoring the care process, documenting and communicating patient status, collecting data to evaluate timeliness and appropriateness of care, collaborating with physicians, nurses, and other health care professionals to provide comprehensive assessment, treatment, and follow-up, and staying informed on health care trends to optimize resource utilization. Requires RN licensure, BSN, and at least 2 years of relevant clinical experience; prior case management experience is strongly preferred.
