Care Manager RN - HEMATOLOGY ONCOLOGY

Conduct shared patient visits with providers and perform independent RN visits using standardized procedures and evidence-based workflows within the Team Based Care model. Utilize the nursing process to care for a group of patients safely, therapeutically, and efficiently while collaborating with team members to coordinate and execute the plan of care. Demonstrate initiative in the delivery of patient care under the direction of the Supervisor/Manager of Clinic Operations and clinical oversight from the Director of Clinical Operations.
