Job Summary
Care Manager RN at St. Mary Medical Center in Apple Valley, CA. Full-time night shift with 12-hour duties. Case Management involves collaboration with patients, caregivers, nurses, social workers, physicians, payers, and the care team to facilitate communication and care coordination along a continuum and through effective transitional care management. Requires California RN license and a Bachelor's degree in nursing; 2 years of healthcare-related experience and 2 years in Case Management (or TIP program) are expected; Master’s degree preferred. Includes potential for shift differentials and additional compensation beyond base pay.
Required Qualifications
- Bachelor's degree in nursing
- California Registered Nurse License
- 2 years of experience in healthcare related field (Acute, Ambulatory, Post-Acute, etc.)
- 2 years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager
Desired Qualifications
