Job Summary
Care Manager RN responsible for coordinating care across patients, caregivers, nurses, social workers, physicians, payers, and the community; facilitates effective transitional care management and collaborative case management processes. Requires CA RN license on hire, with a path to a Bachelor’s degree if not already held, and at least 2 years of healthcare experience including case management or TIP. Prefer Master’s degree and credentials (ACM/CCM). In-person Day shift at Providence Saint Joseph Medical Center in Burbank, CA.
Required Qualifications
- Bachelor's Degree in Nursing (or plan to obtain within 3 years)
- California Registered Nurse License on hire
- 2 years experience in Healthcare related field
- 2 years experience in Case Management or TIP program
