Job Summary
Provide care management and care coordination services for at-risk patients with complex medical and psychosocial issues using population-based, chronic care management approaches and clinical protocols. Serve as a core member of patient-centered medical home teams led by physicians and providers. Maintain Oregon Registered Nurse licensure and three years of clinical RN experience. Full-time day shift role at Providence Health Plaza in Portland, OR.
Required Qualifications
- Graduation from an accredited school of nursing
- Oregon Registered Nurse License
- 3 years Clinical experience as an RN
Desired Qualifications
- Bachelor's Degree in Nursing
- 2 years Home health, skilled nursing facility and/or community health nursing
- 2 years Managed care experience and/or utilization management experience
- 2 years Care/Case Management experience
