Care Manager RN - Case Management

RN provides transitional and chronic care management under the general direction of the Manager, ACM, with clinical oversight from the Director of Clinical Operations. The role involves coordinating care and performing case management within Kadlec Providence, supporting patient-focused, whole-person care in a hospital/clinic setting. Preferred qualifications include a Bachelor's degree in nursing and 2 years of hospital or ambulatory care management experience. The position is full-time, on-site in Richland, WA.
