Job Summary

Conduct comprehensive member assessments to determine care management eligibility based on clinical triggers and psychosocial wellness. Develop and implement individualized care coordination plans in collaboration with physicians, caregivers, and multidisciplinary teams. Perform telephonic, face-to-face, or home visits to monitor care progress, document interventions, and suggest plan adjustments. Facilitate interdisciplinary care team meetings and promote service integration across behavioral health, long-term services, and community resources. Maintain ongoing member caseloads for regular outreach while addressing barriers to care and providing medication reconciliation as needed. Requires active RN license, valid driver's license, and ability to handle 25-40% local travel.

Required Qualifications

  • At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience
  • Registered Nurse (RN)
  • License must be active and unrestricted in state of practice
  • Valid and unrestricted driver's license
  • reliable transportation

  • Understanding of the electronic medical record (EMR)
  • Understanding of Health Insurance Portability and Accountability Act (HIPAA)
