Care Manager RN - Accountable Care

Coordinate care for Pacific Medical Centers patients to assure cost-effective utilization of health care resources. Manage, direct, and coordinate the entire spectrum of care for patients across inpatient and outpatient settings. Assist physicians with management and coordination of care at varied clinic and provider locations. Maintain Washington Registered Nurse License and Certified Case Manager certification. Requires two years of clinical experience, including utilization review and case management.
