Care Manager RN *0.4 FTE Day*

Coordinate complex discharges and develop progressive plans to ensure safe transitions to the next care setting. Actively manage cases by engaging patients and families to prepare them for complex needs, following patients through the continuum of care into different facilities and home. Focus on medication reconciliation between levels of care to reduce avoidable readmissions. Meet with patients at various care levels to provide consistent transition overviews. This role supports the organization's mission of caring for vulnerable communities within the Pacific Northwest healthcare system.
