Care Manager (Registered Nurse/RN)

Job Summary
RN Care Manager coordinates patient care and services in a post-acute setting. Works with patients, families, care teams, payers, and external entities to promote safe transitions from facility to next care setting, ensuring compliance with applicable laws and Life Care standards. Requires an active RN license, nursing diploma (associate or bachelor), strong organizational skills, ability to train and educate patients and families, and experience in care coordination, case management, and discharge planning. Role involves on-call rotation (nights/day as assigned), efficient use of computer systems, and collaboration within an interdisciplinary team to deliver quality, patient-centered care.
Required Qualifications
- Active Registered Nurse (RN) license
- Nursing diploma (associate’s or bachelor’s degree)
- Post-acute care experience preferred
- Case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or RN-BC certification preferred
