Job Summary
RN Care Manager coordinates the patient’s care and services in a post-acute setting, collaborating with patients, families, care teams, payers, and external entities to ensure safe transitions from the facility to the next provider. Requires an active RN license and a nursing degree (associate or bachelor). Schedule includes Tuesday–Saturday with nurse on-call rotation and includes a $3,000 sign-on bonus. Responsibilities include care coordination, utilization review, discharge planning, data entry, and maintaining professional standards and compliance.
Required Qualifications
- Nursing diploma (associate’s or bachelor’s degree in nursing)
- Current active Registered Nurse (RN) license
- One year of clinical experience in post-acute care preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or RN-BC preferred
