Care Manager - Registered Nurse

Job Summary
The Care Manager RN collaborates with a multidisciplinary team to develop and adapt personalized care plans for patients with chronic illnesses, primarily kidney diseases. The role involves performing in-home care management visits, assessing patients’ health, educating them on CKD and treatment options, and coordinating care with medical providers and family support systems. Candidates must have a valid RN license, BLS certification, and at least 2 years of experience in care management or working with CKD/ESRD patients.
Required Qualifications
- Currently licensed as a Registered Nurse in the State of the posted location
- Graduate of an accredited School of Nursing
- Basic Life Support (BLS) certification
- 2+ years previous experience working in care management
Desired Qualifications
- Experience in care management
- Experience with CKD/ESRD patients
- Excellent verbal communication skills
- Familiarity with Microsoft Office and mobile phone applications
