Job Summary
Care Manager Registered Nurse coordinates and advocates for patients across the care continuum. Responsible for comprehensive assessments, development and implementation of individualized care plans, facilitation of transitions of care, and collaboration with physicians, interdisciplinary teams, patients, and families. Monitors outcomes, manages utilization, ensures adherence to evidence-based guidelines, and promotes patient education and self-management in a patient-centered, collaborative environment that supports professional growth and advancement.
