Job Summary
Advocate for members with intellectual and developmental disabilities to ensure informed decision-making and appropriate guardianship. Schedule and lead interdisciplinary meetings to develop comprehensive, person-centered Life Plans that reflect individual goals. Implement, update, and monitor these plans while facilitating reviews and approval processes. Integrate all necessary supports including medical, behavioral, and community-based services. Maintain ongoing contact with critical people in a member's life and assist with benefits like Medicaid and Medicare. Resolve living problems such as housing, utilities, and safety issues. Ensure timely submission of all documentation and report abuse or neglect immediately. Perform required home visits and attend periodic office trainings in a hybrid environment.
Required Qualifications
- A Bachelor's degree with two (2) years of relevant experience
- A License as a Registered Nurse with two years of relevant experience
- A Master's degree with one year of relevant experience
- Strong communication skills, including verbal and written communication skills
- Strong interpersonal and organizational skills
- Excellent organizational, interpersonal, and verbal and written communication skills
- Bilingual in English and Spanish
