Care Manager

Coordinate patient care and manage discharge planning to ensure timely and safe transitions for all assigned cases. Progress care for patients within the acute or post-acute setting while expediting discharge processes. Maintain current unrestricted registered nurse licensure in Georgia and utilize InterQual or Milliman systems as needed. Preferred qualifications include a Master's degree, two years of relevant experience, and certification in case or care management. This role supports the care coordination team at Piedmont Atlanta Hospital.
