Job Summary
Conduct client visits in homes, assisted living facilities, skilled nursing facilities, physician offices, and hospitals to assess physical, cognitive, emotional, and environmental needs. Complete daily logs in Excel and MyJunna with accurate billing entries, pill box checks, and required photographs by end of business day. Prepare weekly client schedules in MyJunna and Outlook by Friday noon, recheck on Sunday evening, and check in with the supervisor daily. Communicate weekly with families and as needed with physicians, aides, and care team members; escalate changes in condition, safety risks, or care-plan gaps promptly. Coordinate services across providers and ensure safe transitions between care settings. Participate in care conferences, family meetings, and case reviews as assigned. Maintain strict compliance with HIPAA and company policies while driving throughout Palm Beach County to client locations.
Required Qualifications
- Minimum of 3 years of experience in care management, eldercare, hospice, social work, or complex senior care coordination
- Valid Florida driver's license
- Reliable personal vehicle
- Current personal auto insurance
- Ability to travel locally between multiple client locations each day throughout Palm Beach County
- Dedicated home office
