Job Summary
Care Manager for Health Home Care Management located in Peekskill, NY, coordinating the care team for patients with complex medical and psychosocial needs to ensure access to medical and social services. Works with interdisciplinary teams, directs field activity with the Patient Navigator, develops and updates individualized care plans, identifies barriers to care, ensures medication reconciliation, participates in discharge planning and post-discharge follow-up, arranges preventive and metabolic screenings, facilitates communication between patients and providers, and utilizes the TREAT system for timely documentation and assessments. Requires at least a high school diploma or GED and 2 years of relevant work experience; a Bachelor's degree in Health or Human Services is preferred.
Required Qualifications
- Minimum Education: High School diploma/GED
- Minimum Work Related Experience: 2 years
- Ability to coordinate care with interdisciplinary team
- Experience with discharge planning and care transitions
- Experience with patient education and self-management support
- Proficiency in scheduling and documentation systems
- Strong communication and collaboration skills
- Bachelors degree preferred in Health or Human Services (not required)
