Care Manager- New Britain/Hartford Team

Job Summary
Care Manager coordinates Long-Term Services and Supports for individuals, conducts comprehensive person-centered assessments (in preferred settings), educates clients on program components and options, processes required documentation, monitors care plans and renewals, collaborates with an interdisciplinary team, participates in on-call coverage, and may mentor staff. Requires a BA degree and at least two years of direct case management experience; bilingual ability preferred. Location-based in Bristol, CT; role involves interaction with clients and families, documentation, and coordination of community resources to support consumer choices and rights.
Required Qualifications
- Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field
- Minimum of two years’ experience in health care or human services
- Bachelor’s degree in Nursing, Health, Social Work, Gerontology or related field may substitute for one year of experience
- Knowledge of Medicaid and Medicare
- Strong advocacy and communication skills
- Familiarity with community resources
- Reliable transportation and valid driver’s license
- Possession of automobile insurance
- Ability to perform home visits as required
Desired Qualifications
