Job Summary
The Care Manager coordinates formal and informal caregivers to support clients’ informed decision-making around Long-Term Services and Supports. Responsibilities include conducting person-centered assessments in the client’s preferred setting, educating individuals on program components and eligibility, promptly completing documentation, monitoring and adjusting care plans, and collaborating with an interdisciplinary team. The role may involve mentoring new staff and participating in on-call coverage. Requires a BA degree and at least two years of direct case-management experience; bilingual Spanish preferred. Location-based duties center on serving clients in the Naugatuck/New Britain region (Watertown, CT area referenced).
Required Qualifications
- Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required
- Must have a minimum of two years’ experience in health care or human services. A Bachelor’s degree in Nursing, Health, Social Work, Gerontology or a related field may be substituted for one year of experience
