Care Manager MSW Per Diem Day

Conduct professional, comprehensive patient-centric care management for at-risk patients in an acute care environment, including assessment, planning, and discharge coordination. Coordinate care, document interventions, ensure regulatory compliance, and advocate for patients by identifying resources to address social, financial, cognitive, or legal barriers. Maintain continuity of care by connecting vulnerable patients with needed services. Requires Master's degree in Social Work, one year of healthcare experience, and acute care or case management orientation experience. Work 8-hour day shifts as per diem.
