Job Summary
Assess and plan comprehensive care for at-risk patients in an acute care environment, coordinating services and managing discharge planning. Document interventions, ensure regulatory compliance, and advocate for vulnerable patients by identifying resources to address social, financial, cognitive, or legal barriers. Maintain continuity of care by connecting patients with needed support services. Requires a Master's degree in Social Work from a CSWE-accredited program and one year of healthcare experience, including acute care exposure or completion of TIPs/Care Management Orientation. Full-time, 8-hour day shifts.
Required Qualifications
- Master's Degree in Social work from an accredited CSWE program
- 1 year Social Work experience in any healthcare setting
- 1 year Experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
Desired Qualifications
- Basic understanding of acute medical surgical healthcare terminology
- prior case management experience
