Job Summary
Build trusted relationships with eligible priests through regular outreach and personal visits, serving as their primary point of contact for navigating healthcare and benefits. Coordinate medical, dental, and specialist appointments while assisting members in accessing third-party services such as DME, home health, and transportation. Conduct support visits during hospitalizations and post-hospitalization follow-ups to coordinate discharge planning, identify home safety concerns, and assist with non-clinical tools like alert systems. Collaborate with pharmacists, nutritionists, and social workers to ensure comprehensive support, maintaining accurate documentation in accordance with HIPAA and privacy protocols. This role blends purpose-driven service with professional expertise to help eligible priests access coordinated resources through non-clinical care navigation and advocacy.
Required Qualifications
- Minimum 3 years of healthcare experience
- Must successfully complete background checks and all required screenings
- Ability to work independently while collaborating with healthcare professionals and the Meta Care team
- Proficiency in Microsoft Office (Word, Excel, PowerPoint)
- Demonstrated commitment to performing the role in a manner consistent with Catholic Social Teaching and the Ethical and Religious Directives for Catholic Health Care Services, including respect for human dignity, care for the vulnerable, and fidelity to the moral teachings of the Roman Catholic Church
Desired Qualifications
- Preferred LPN or RN (active license not required)
- Bi-lingual preferred (Spanish)
- Experience in clinical or medical office settings with a passion for helping individuals navigate the healthcare system through advocacy, education, and care coordination
- Exceptional communication and listening skills with the ability to offer emotional support
- Strong organizational abilities and attention to detail
