Care Manager - LVN

Care Manager coordinates care for SCAN members in collaboration with the Integrated Care Team to ensure high-quality, timely, and coordinated care, directing members to appropriate facilities and arranging necessary services, supplies, and information. Responsibilities include authorizing services based on utilization-management and medical-necessity guidelines, conducting pre-service and concurrent reviews, coordinating discharge planning from inpatient settings to skilled nursing facilities, ensuring timely delivery of last cover day letters, coordinating services such as home health and DME, reviewing appeals, preparing detailed explanations of non-coverage, and collaborating with Care Coordinators and Medical Directors. The role requires an Associate’s Degree or equivalent experience, CA LVN license, and 3-5 years of related experience. The position supports a remote work mode with a home office in California and offers a base hourly rate in a remote employment arrangement.
