Job Summary
Care Manager for LTSS with field travel in Kenosha County, WI. Provides comprehensive care management and care coordination for members with high-need potential, including in-person home visits, waiver enrollment/disenrollment, and development/monitoring of waiver service plans in collaboration with members, families, physicians, and care teams. Performs assessments to determine medical necessity and authorize waiver services, facilitates ICT meetings, and supports integration of behavioral health and LTSS with community resources. Role requires 25-40% local travel and strong skills in motivational interviewing, coordination across disciplines, and effective communication to address psycho/social, financial, and medical barriers.
Required Qualifications
- At least 2 years health care experience
- At least 1 year experience in care management or LTSS
- 1 year of experience in medical and/or behavioral health setting or equivalent
- Valid and unrestricted drivers license
- Reliable transportation and adequate auto insurance for job-related travel
- Demonstrated knowledge of community resources
- Ability to work independently with minimal supervision
- Excellent time-management and prioritization skills
- Strong verbal and written communication skills
- Microsoft Office proficiency or applicable software/navigation of online portals
