Job Summary
Care Manager - LSW at Longview Regional Medical Center coordinates discharge planning, transitions of care, and case management in an acute hospital setting. Responsibilities include reviewing medical records for appropriateness and medical necessity, collaborating with interdisciplinary teams, developing discharge plans, referring cases to physicians/RNs as needed, liaising with community agencies, leading interdisciplinary meetings, maintaining documentation, and ensuring compliance with federal/state standards. Required Qualifications
- Bachelor's Degree in Social Work required

- 2-4 years of experience in an acute care hospital setting required
- LCSW - License Clinical Social Worker required or LMSW - Licensed Medical Social Worker - State Licensure required
