Care Manager - LP (Henderson County, NC)

Care Manager - Licensed Professional provides proactive intervention and coordination of care across MH/SU/I/DD/TBI and physical health to ensure appropriate assessment and services. Works with members, care team, and caregivers to identify care gaps, conducts comprehensive HRAs, administers screenings (PHQ-9, GAD, ACES, LOCUS/CALOCUS), develops and updates person-centered Care Plans, and coordinates services across health domains. This mobile role requires travel and in-person contacts in members’ home communities; must reside in or near Henderson County, NC, and meet NC residency requirements. The role leverages advanced clinical knowledge to support care planning, crisis management, transitional care, Medicaid eligibility, and collaboration with interdisciplinary teams within Vaya’s integrated care model.
