Job Summary
Care Manager coordinates access to services for individuals with intellectual and developmental disabilities, developing and monitoring person-centered Life Plans, advocating to meet members’ goals, and coordinating interdisciplinary team efforts. Responsibilities include completing assessments/reassessments, identifying gaps in services, developing and implementing Life Plans within required timeframes, ensuring documentation and billing accuracy, maintaining eligibility and enrollment in HCBS, linking to benefits (Medicaid, Social Security, SNAP), coordinating healthcare services and preventative care, facilitating transitions from inpatient settings, promoting community inclusion, utilizing electronic health records and telehealth, supporting member self-advocacy, traveling to member homes and communities, and maintaining privacy standards in flexible work environments. The role requires a valid NY driver’s license and a dependable vehicle, willingness to travel, and ability to work in a blend of home, field, and office settings in the designated service area (Ithaca, NY and surrounding region); evening/weekend flexibility may be required.
Required Qualifications
- Bachelor of Arts or Science with two years of relevant experience, or RN license with two years of relevant experience, or master's degree with one year of relevant experience
- Degrees in Health and Human Services, Psychology, Sociology, or related fields preferred
- Must have valid Driver’s License from NY or a contiguous state and dependable vehicle for travel
- Able to meet members in homes or community locations; able to travel to off-site locations
