Job Summary
Care Manager to provide outreach and enrollment services to children enrolled in New York State’s Health Home programs (EI, CPSE, CSE, and related services). Roles include documenting case activity, administering assessments (CANS-NY), developing a comprehensive Plan of Health Care, coordinating services, and delivering family-driven, youth-guided services across home-, school-, center-based settings or at Liberty POST locations. Responsibilities include educating families, helping secure public benefits, delivering Health Home services (at least two services per month with at least one in person), and ensuring smooth transitions of client services. Bilingual candidates are encouraged; Spanish language skills are a plus. Benefits include health insurance for full-time positions, mentoring, CEU credits through ASHA/AOTA/APTA/CTLE, flexible scheduling, and biweekly pay. Job types available: Part-time and Full-time; starting salary at $45,000 for full-time roles.
Required Qualifications
- BA with 2 years relevant experience.
