Job Summary
Manage a caseload of members with moderate to complex needs by conducting assessments, community-based outreach visits, and developing individualized care coordination plans. Coordinate medical, behavioral health, and community services while supporting discharge planning and transitions of care. Educate members and families on their care options and document all activities accurately within electronic systems. This hybrid role requires an active DC LICSW license and Master's degree in Social Work, with Spanish preferred. V-Tech Solutions serves children and young adults with special needs through comprehensive care coordination.
Required Qualifications
- Master's Degree in Social Work or related field
- Active DC LICSW license
- 3+ years care management or community health experience
- Strong communication and documentation skills
