Care Manager II

Care Manager II conducts comprehensive assessments of clients with multiple medical and psychosocial needs, develops and initiates the Community Living Plan with client input, performs home visits and escorts to appointments, coordinates community services (Medicare/Medi-Cal and entitlement programs), provides crisis intervention and advocacy, maintains ongoing client relationships with the informal support network, documents care management activities and updates the Community Living Plan within 24-48 hours, collaborates with physicians and other health professionals, and supports continuous quality improvement within geriatric and community health care settings.
