Job Summary
Responsible for coordinating services for youth and adults in community settings to reduce emergency room visits and hospitalizations. The Care Manager II conducts comprehensive assessments, engages members, and collaborates with a multi-disciplinary team to create and evaluate individualized service plans. Key responsibilities include facilitating team meetings, maintaining communication with members and community service providers, and managing a large caseload. A Bachelor's degree and a minimum of 2 years experience in human services are required.
Required Qualifications
- Bachelor's degrees
- Minimum 2 years of experience working in a human services or related position supporting youth, adults and families
- Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record
- Must receive CANS certification score of 70 within 3 months of hire and annually thereafter
