Job Summary
Conduct comprehensive and ongoing assessments of clients with multiple medical and psychosocial needs, including physical, mental, and spiritual requirements. Develop client-centered Community Living Plans based on assessment data and coordinate home visits, hospital visits, and medical appointments. Monitor community services aligned with Medicare and Medi-Cal entitlements while providing crisis intervention, advocacy, and therapeutic support. Meet clients monthly to review care plans, document activities within 24-48 hours, and evaluate Purchase of Service needs. Collaborate with physicians, caregivers, and team members to ensure continuity of care, coordinate placements, and promote continuous quality improvement. Maintain open communication with providers and participate in team meetings, training, and problem-solving endeavors.
Required Qualifications
- B.A or B.S in Social Work or related field
- One year working with disabled adults and/or older adults
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults
- Exceptional communication and presentation skills relating to the functionally impaired adults and older adults, their support systems and teams of health professionals
- Demonstrates case management skills and experience in the community health care delivery system
- Detail oriented with good problem solving skills and the ability to prioritize multiple tasks
- Computer literacy
Desired Qualifications
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders
- Bilingual English/Spanish
