Job Summary
Care Manager I (CMI) conducts comprehensive psychosocial assessments of residents, coordinates and monitors community-based services for low-income older adults, develops a client-centered Community Living Plan, educates clients and families about resources, maintains detailed documentation, collaborates with physicians and health providers, participates in CQI initiatives, and attends IOA/Santa Clara Methodist Retirement Foundation meetings. Requires a Bachelor's degree in Social Work or related field and one year of relevant experience; bilingual ability (English plus Cantonese/Mandarin/Spanish) strongly preferred; compensation: $72,000–$74,000 annually; location: Fairfield, CA; hybrid/remote options not indicated; physical ability to lift up to 25 lbs; strong communication and organizational skills.
Required Qualifications
- B.A or B.S in Social Work or related field required
- One year’s experience working with disabled adults and/or frail elderly required
- Bilingual highly preferred; English plus Cantonese and/or Mandarin and/or Spanish
- Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems and teams of health professionals
