Job Summary
Lead Health Home Care Manager coordinating care for a caseload of ~60 chronically ill patients; conducts initial assessments and develops patient-centered care plans, collaborates with the patient and care team to implement plans, performs home/field visits, coordinates services with internal and external providers, documents encounters, updates plans with outcomes, and provides advocacy and referral services to address medical and psychosocial needs.
Required Qualifications
- LPN/MSW/MPH/BA/BS Degree is required
- Two (2) years experience in care coordination is preferred
