Job Summary
Provide Coordinated Specialty Care (CSC) model to families and consumers with severe mental illness and support team members. Responsibilities include organizing and executing team case conferences, participating in FEPI on-call rotation and meetings, engaging with consumers monthly to assess needs and develop care plans, conducting community visits (home, school, court, etc.), addressing mental/physical health and social determinants of health, coordinating care and referrals to community resources, advocating for consumers, collaborating with support systems, ensuring timely documentation, meeting productivity standards, and participating in supervision and professional development. Compliance with HIPAA and agency policies, along with ongoing participation in trainings and mandatory in-services, is required. The role emphasizes leadership through example and building collaborative relationships across departments and with community partners, aligned with Integrated Care Core Values of connection, innovation, compassion, and integrity.
Required Qualifications
- Bachelor’s Degree in a health-related field of counseling, psychology, social work or other behavioral health field
- Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health
- Valid Tennessee Driver’s License
- Acceptable Motor Vehicle Report (MVR)
