Job Summary
Care Manager responsible for ensuring client caseloads receive necessary services through coordinated and effective delivery of case management, advocacy, and supportive services to ensure coordination of care for clients. Requires HS diploma or GED plus 4 years in Behavioral Health (or combined education/experience) and preferred 1 year of case management experience. Certifications in CPR/First Aid and Crisis De-escalation, valid driver’s license, fingerprint clearance, TB screening, and ability to work evenings. Bilingual English/Spanish and experience in Integrated Healthcare/Behavioral Health/Primary Care are valued. Ability to lift up to 50 pounds and operate agency vehicle. Demonstrates strong communication, organization, and problem-solving skills.
Required Qualifications
- HS diploma or GED
- 4 years in Behavioral Health or related education/experience
- One year case management experience
- Certification in first aid, CPR and Crisis De-escalation
- Must have valid driver’s license with acceptable driving record and reliable transportation
- Willingness to work evenings as needed
- Must obtain fingerprint clearance card and pass drug/background checks
- TB screening prior to providing services and annual TB follow-up
- CPR/First Aid Certification (training provided if not possessed)
- Ability to perform clinical tasks safely
- Organizational abilities for coordination with clinical team
- Excellent verbal communication
- Ability to analyze variables and situations
- Physical ability to sit long periods, bend, lift up to 50 pounds
- Operate phone, fax, copier, computer and agency vehicle
- Bilingual English/Spanish (preferred)
- Experience in Integrated Healthcare/Behavioral Health/Primary Care (preferred)
- Cultural diversity sensitivity (preferred)

- TB screening evidence of freedom from infectious tuberculosis prior to providing services

- Physical, mental, and cognitive ability to perform required clinical tasks without posing risk to patients
- Demonstrated organizational abilities to maintain communication and coordination with clinical team members
- Bilingual – English/Spanish (preferred)
