Job Summary
Care Manager leads Health Home Plus (HH+) care coordination for HIV-focused patients, conducting biopsychosocial assessments, developing patient-centered plans of care, performing face-to-face assessments in patients' homes, providing crisis intervention and HIV prevention/treatment education, coordinating with internal and external providers, and ensuring adherence and retention in care across approximately 15–20 patients. Responsibilities include home/field visits, care conferencing, documentation in patient charts, utilization of registry/EHR systems, and participation in QA/QI projects. Requires a Masters or Bachelor's degree in related fields and relevant experience with HIV, mental illness, homelessness, or substance use.
Required Qualifications
- Masters or Bachelor’s Degree in Human Services, Education, Social Work or Mental Health is required with at least one (1) year of experience working with target populations defined as individuals with HIV, history of mental illness, homelessness or substance abuse.
- Associates Degree in Health, Human Services, Education, Social Work or Mental Health with at least two (2) years of experience working with target populations defined as individuals with HIV, history of mental illness, homelessness or substance abuse.
