Job Summary
Care Managers provide outreach and enrollment services to children presumed to meet New York State Health Home program eligibility, documenting activities and coordinating plans of care across families, schools, and service providers. Responsibilities include administering assessments (CANS-NY), developing comprehensive Plan of Health Care, coordinating existing and additional services, conducting quarterly reviews, educating families about services and conditions, assisting with obtaining public benefits, and delivering Health Home services with child-centered, family-driven, youth-guided approaches. Position supports in-home, school-based, or center-based service delivery in Goshen, Wappinger, Sugar Loaf, or Ulster County, NY. Bilingual candidates encouraged; Spanish proficiency is a plus.
