Job Summary
Health Homes Care Manager (field-based) in Bronx & Manhattan, NY. Performs outreach to engage eligible patients for enrollment in the Health Home Care Management Program, conducts comprehensive assessments, develops patient-centered care plans, and coordinates care with providers, pharmacies, insurers, and caregivers. Duties include screening for Health Home eligibility, managing referrals and access to local resources, providing crisis intervention when needed, conducting home visits, and maintaining documentation in Health Homes standards. Requires an Associate's degree, 2 years of care management experience, and ability to travel across Bronx and Manhattan for in-home patient visits. Familiarity with data systems (e.g., Salesforce) and strong communication skills are essential; bilingual Spanish is a plus. Field-based role with travel to patients’ homes in NYC.
Required Qualifications
- Associate's degree is a must
- 2 years of Care Management experience
- Must be able to travel to minimum of 2 boroughs - Bronx and Manhattan
- Ability to travel in the field to accompany patients to appointments and meet patients in person when needed
- Strong working knowledge of local community resources
- Demonstrated ability to work with data reporting, documentation, and outcomes
- Strong communication and assessment skills; the ability to relate to patients, their families, and community care providers
