Job Summary
The Care Manager (HARP) coordinates care for patients with complex medical and psychosocial needs, developing and managing coordinated care plans with clients and their families, and collaborating with interdisciplinary teams. Responsibilities include conducting clinical assessments, monitoring patient progress, maintaining case records, and participating in case conferences and meetings. A Bachelor's degree in Health or Human Services and two years of relevant work experience is required, with a Master's degree preferred. Candidates must possess strong communication skills and the ability to work under pressure.
Required Qualifications
- Bachelor’s degree in Health or Human Services related field
- 2 years of related work experience
- Ability to communicate well with medical providers and support staff
- Ability to work well with diverse population
- Ability to handle multiple tasks and stressful environment
- Ability to work well within the organizational structure
- Ability to work flexible hours including some evenings and weekends
- Ability to use common office software (Word, Excel, Power Point)
- Effective oral/written/interpersonal communication skills
