Job Summary
Care Manager - GSSC oversees a caseload of older adult consumers, determines eligibility for home programs, conducts home visits, assesses needs, develops service plans, and maintains records in alignment with EOEA standards. Responsibilities include functional assessments, coordinating services through contracted vendors, monitoring service plans, performing SCO program duties, ensuring documentation accuracy, and collaborating with agency teams. Requires a Bachelor's degree, experience with the elderly preferred, valid driver's license, and a reliable car. May participate in CTLP activities and maintain compliance with HIPAA/PII policies.
Required Qualifications
- Bachelor's Degree required, preferably in a social service-related field.
- Experience utilizing MS Office including Word and Outlook for email and calendaring.
- Experience working with the elderly is preferred.
- A reliable car is required, as well as a valid driver's license.
