Job Summary
Develop comprehensive care plans for seniors requiring medical and cognitive support, including continuous assessment of client conditions, preferences, and medical orders. Track and document observations, monitor client status and interventions, and report outcomes to ensure dignity and independence. Coordinate resources and interact with physicians, attorneys, and community personnel to advocate for clients and families. Manage electronic health records, including billing, and create referral resources while providing guidance for informed medical decisions. Travel locally by personal automobile to client homes to deliver personalized care management.
Required Qualifications
- (5) Years' experience in community, hospital, hospice or home health case management
- Certified Care Manager (CMC) or Certified Case Manager (CCM)
- Demonstrated knowledge of aging, dementia, and/or disabilities
- Experience working with complicated family systems and dynamics
- MS Office
- Electronic Health Records
- Internet Skills
- Excellent customer service and problem-solving skills
- Excellent verbal and writing skills
- Detail-oriented with the ability to keep up with fast paced processes
- Ability to lift and carry up to 10-15 pounds
- Ability to sit, stand and walk for prolonged periods of time throughout the work day
- Frequent local travel by personal automobile to client homes required
- Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
- timely and accurate positive fingerprint identification of California based applicants
Desired Qualifications
- Bachelor's or master's degree in Healthcare Related Field, preferred
